最近一位朋友和肝胆胰脾外科的副主任焦华波聊起自己的父亲——一位70岁的老先生。这位老先生几年前因为胆结石把胆囊切掉了，切完了以后总觉得隐隐作痛，但又不是剧烈的疼。这位老先生也是一位很有文化的知识分子，在网上查资料，查看书籍之后，越看越觉得自己有后遗症，认为自己有可能是胆囊切掉之后，胆管里面又长了石头，后悔把胆囊切了。那这位老先生胆管里面究竟有没有结石？如果患有胆结石的话，到底要不要切除胆囊呢？医院医院肝胆胰脾外科副主任焦华波今天就给大家指点迷津！详细的讲讲胆囊的这些事。
胆囊在人体中起的作用
如图所示，胆囊是位于肝脏后方的梨形囊袋构造。胆囊经胆囊管汇入胆总管。胆总管和胰腺管相交（85%的人是汇在一起的，还有15%的人是胆管和胰管各自开口在十二指肠上面）进十二指肠。肝产生的胆汁经肝内胆管排出之后再经胆总管进入肠道里面帮助油脂类食物消化。没有胆囊或胆囊因病丧失功能，胆汁也可直接排入肠道参与消化功能。像高速公路旁的“加油站”一样的胆囊对胆汁起到贮存、浓缩、收缩、排空的作用，因此胆囊被称为“胆汁仓库”。胆管里面的胆汁看起来是很稀很透明的，外观就像豆油一样，而胆囊里面的胆汁很稠、颜色很深。当你进食油水很大的时候，胆囊就会收缩排出胆囊内胆汁，浓缩的胆汁进入十二指肠能更好地帮助油脂消化。因为胆囊里的胆汁是浓缩的胆汁，所以胆囊里排出的胆汁会比肝产生的胆汁效率更高。

切完胆囊之后的副作用有多大？
胆囊对我们的影响究竟有多大？这跟我们日常吃的食物种类有关系。在日常饮食里，吃点五谷、蔬果、乳类、肉类（瘦肉，鱼、虾），这时候胆囊对我们的消化系统影响并不大。当我们进食的油脂特别重的时候，如吃大肥肉，大碗的红烧肉等，这就需要更多的胆汁排出帮我们消化，有胆囊的人消化能力就会强。所以切完胆囊之后的副作用是什么？最大的副作用就是高脂餐饮之后的腹泻，俗称拉肚子。
在这给大家讲一个有趣的事：有些食草动物像马、鹿、驴、大象是都没有胆囊的。
现在我们已经知道胆囊对我们的影响了，当我们患有胆结石的时候，胆囊要不要切除，就要权衡它的利与弊了。
患有胆结石时，胆囊还有它的作用吗？
当胆囊装满石头的时候，因为胆囊内没有空间再装胆汁，这时胆囊也就起不到贮存胆汁的作用了。
胆结石引起慢性胆囊炎时，胆囊的浓缩和收缩功能也会下降。 有时候胆囊炎症很重或者胆囊管被结石堵塞，胆囊的排空能力没有了，它也排不出胆汁了。
总结：当患有胆结石时，胆囊的贮存、浓缩、收缩和排出功能变差了，此时它（胆囊）的存在可能弊大于利了。
胆结石不及时治疗危害大
1胆囊里面有石头，石头堵在胆囊管里，病人容易犯急性胆囊炎。
2胆囊里面的石头掉进胆总管里，就会得急性梗阻性化脓性胆管炎，病人除有腹痛症状外，还常有寒战、高烧和黄疸。急性梗阻性化脓性胆管炎的危害比胆囊炎大得多，严重时会危及生命。
3由于85%的人胆总管和胰腺管相交，所以胆囊里的石头排出来，一旦进入到胰腺里胆胰管汇合部，就会得急性胰腺炎,急性重症胰腺炎严重时甚至会导致死亡。
总结：胆结石长期刺激胆囊壁引起胆囊炎，不及早治疗还容易掉入胆总管诱发胆管炎、甚至胰腺炎。长期的慢性炎症刺激还可能导致胆囊癌的发生。
那患有胆结石的患者究竟该怎么治疗呢？切除胆囊？排石？溶石？
患有胆结石，排石行不行？
对于胆囊炎有些人主张碎石，让石头排出去，焦华波主任指出这种做法风险太大！不主张碎石的原因就是因为，石头碎了更容易进入胆总管，排石治疗会有两个关卡。这两个关卡就是胆囊管和胆胰汇合部到十二指肠口，石头卡到胆管和胰腺没有排进肠道，后期更难治疗，所以碎石、排石的风险太大。他举了一个以前看到过的例子，一个病人胆囊里有四颗石头，排掉三颗了，第四颗卡在胰腺那里，得了胆管炎和胰腺炎，最终还是急诊做手术了。
胆结石的治疗应避免哪些误区
对于治疗结石性胆囊炎的误区，焦华波主任总结了四点：
1胆囊对人有多重要？
胆囊对消化功能有作用，但它的重要性没有大家想的那么重要。如果得了胆囊炎，甚至是胆囊功能废掉了，还把它留在那里是没有必要的。此时，弊大于利。
2胆囊结石里的石头是越小越好吗？
胆结石一般人认为石头越大越不好，但医生一般担心小石头。大石头就祸害一个胆囊，小石头掉出来就祸害整个肝，跑到胆管里会得急性梗阻性化脓性胆管炎，甚至还祸害胰腺，诱发急性胰腺炎。急性化脓性胆囊炎即使穿孔也不一定致命，除非是岁数特别大、本身又有严重心肺脑疾病的。但急性梗阻性化脓性胆管炎，急性重症胰腺炎都可能致命。所以小结石比大结石风险更大，这也是专家不主张碎石和排石治疗的原因。
3切了胆囊之后，胆管里面还会长石头吗？
切了胆囊后，胆管里一般不会再长石头。事实是胆囊结石治疗晚了，胆囊的石头掉进去胆管了。此时胆囊治好了，但胆管已经形成慢性炎症了，才会反复长石头了。
4市面上经常有一些溶石、排石的药，效果究竟怎么样？
到目前为止市面上还没有特别好的溶石、排石的药。不是急性发作时的保守治疗，无异于不治疗，只是心理安慰罢了。
最后解答下文章开头那位老先生的疑问，胆管里面有石头的话，除了腹痛、还会有高热、黄疸现象。老先生并不是剧烈的腹痛（焦虑也会引起腹部隐痛症状）同时又没有高热和黄疸现象，所以大可不必担心了。
